Membership Applications

Completed Adult and Youth Applications are necessary for you to participate in Boy Scouts.

Health and Medical Record

Annual Health and Medical Record Parts A, B, and C  Parts A and B are completed annually by participants in all Scouting events.  Part C is for the physical exam that is required for participants in any event that exceeds 72 consecutive hours, for all high-adventure base participants, or when the nature of the activity is strenuous and demanding.

Eagle Scout Service Project Workbook and Rank Application

Eagle Scout Service Project Workbook This workbook includes valuable information that can help ensure a Life Scout’s success in the journey toward the Eagle rank. It includes four forms: a proposal, a final plan, a fundraising application, and a project report. Once complete, Eagle Scout candidates must complete the current version of the Eagle Scout Application formImportant note: The workbook and application must be downloaded and filled out by hand or electronically on your computer (control-click the link and “Save link as…”) and opened in Adobe Reader or Acrobat.
